Athletic Packet

All KAMS students who participate in our team athletics must complete the athletic packet and have a current physical on file. The KAMS packet contains the required information that must be turned in to the office. To view the athletic handbook and Player and Parent Code of Ethics, please see below files. Please note, a copy of the front and back of a student's insurance card is required.
